Some people receive healing to the wounded soul through teaching and prayer during the Healing Seminars but require individual spiritual counseling. Those who are looking for deep level of spiritual healing need to provide the information asked for in the following questionnaire. The information will enable us to identify the root cause of the problems and to help the person to deal with the issues allowing the Holy Spirit to bring in the total healing, restoration and deliverance by the authority of the Word of God and prayer. The questionnaire form and the Release Agreement may be printed and be contacted to schedule an appointment for personal spiritual counseling. The personal information provided for deep level of spiritual healing purpose will be treated as strictly confidential.
